Menopause usually occurs in women in their late 40s to early 50s. Menopause is premature if it occurs before the age of 40. Premature menopause is not a normal phenomenon; it is associated with many common diseases that are prevalent in our society. Having premature menopause means Your fertility will be reduced due to the loss of eggs from the ovaries Your bone health will be reduced and you can develop osteoporosis You are at an increased risk of heart disease Symptoms of menopause include hot flushes, vaginal dryness, reduced duration of periods, reduction in sex drive and mood changes As you can see, the symptoms of early menopause can be vague; most women will not experience symptoms until the function of the ovaries has become very very reduced. At this point, it becomes very difficult to provide options for fertility and child-bearing. We, therefore, strongly recommend that all women who are AT RISK of premature menopause should have their ovarian function tested 1-2 times per year, and report immediately to a fertility specialist if they develop symptoms of menopause. Some conditions that increase your risk of premature menopause (also known as premature ovarian insufficiency or failure) include Autoimmune disease – this is one of the commonest causes in our society. Many women know they have autoimmune disease (for example, Hashimoto thyroiditis, Graves’ disease of thyroid, ankylosing spondylitis, SLE, autoimmune kidney disease, type 1 diabetes, Myasthenia gravis, Rheumatoid arthritis, multiple sclerosis, psoriasis); HOWEVER, none of the women with autoimmune disease who were diagnosed elsewhere and reviewed at ICSI or Medicsi were aware that their disease can cause premature menopause. Endometriosis – this is a well-documented cause of premature menopause Chromosomal and genetic disease Premature menopause related to treatment – this includes chemotherapy, radiotherapy, and surgery on ovaries Premature menopause in your mother or sister – studies have shown that the tendency of a women to develop early menopause is significantly affected by her mother’s age at menopause How to monitor your ovarian function Ovarian function is assessed by the following blood tests for hormones. Serum FSH and LH – these must be done on day 2-5 of your periods. If they are done later than this the results are not reliable for diagnosing menopause Serum AMH – this is a marker of how many eggs your ovaries have left in ‘reserve’; this test can be done on any day of your cycleThere is a possibility for cysts of the ovary to interfere with these tests. If your ultrasound shows ovarian cysts, please discuss your blood test reports with a fertility specialist. NOTE: do not wait for a doctor to prescribe these tests for you. If you are known to have autoimmune disease, please get your hormones done at regular intervals. What treatment options are available for women with premature menopause? For Bone and Heart Health – women have the option of starting HRT (hormone replacement therapy), which replaces the female hormones that were naturally being produced by your ovaries before menopause. These hormones include oestrogen and progesterone and play a protective role in bone and heart health. For Fertility – women have the option of preserving their fertility by freezing their eggs or embryos. This option is available to all women who have reached puberty, including unmarried women as they can freeze their eggs (which can be fertilised later on once she gets married). This option is also suitable for those who have been diagnosed with cancer and are due to start chemotherapy or radiotherapy – in such cases, urgent egg/embryo freezing is done so that cancer treatment is not delayed.It is also possible to remove a small piece (less than 1cm) of your ovary and freeze it – this is known as ovarian tissue freezing. This is not currently available in Pakistan, but liaison can be made with fertility clinics abroad to avail this option. You will be in the recovery room at first, before being transferred to the room or ward. Depending on your procedure you might be staying in the hospital for a few hours to a few days after your operation. At Medicsi, we strive to take care of your baby and support the mother and family in the best possible way.   At the time of the baby birth, one of the nursery staff is there to receive the baby. The umbilical cord is clamped and cut near the navel. We perform delaying cord clamping after 60 seconds which has several benefits for your baby.  If your baby is doing well, it will be placed skin-to-skin on the mother’s tummy or chest. Placing baby skin-to-skin with you will help baby adjust to being outside of the uterus, promote bonding between mother and baby, and help regulate baby’s temperature. Under the circumstances; that the baby needs help with breathing he or she will be taken immediately to the Nursery for active resuscitation. Our nurses will support and guide the mother with breastfeeding as soon as possible after the delivery of your baby. The early initiation of breastfeeding helps with the bonding between you and your baby. The baby will be brought to you for the first feed after your delivery process is complete which usually takes about 20 min after the birth of the baby. After the initial mother feed, your baby will be kept in observation for a couple of hours in the nursery where a head-to-toe assessment is performed by a paediatrician to ensure the baby is well. If no health concerns are found the baby is shifted to the room with the mother. In case of any unexpected observations that warrants close monitoring, the baby will be kept in the nursery till needed. Baby is given the following vaccinations in the nursery soon after birth; Hepatitis B vaccination and  Vitamin K injection.  In the meantime, the father or the next of kin are provided with the necessary information regarding the baby and any management that is being done with the baby.  Once the mother has been shifted to the room from the labour ward area the baby will accompany her to the room. Once in the room, relevant guidance is given on breastfeeding, burping, bathing the baby, and temperature control.  If the baby is in hospital, the baby is bathed 24 hours after delivery.  Before discharge, the consultant visits the baby in the room, during the morning rounds, and a repeat head-to-toe health assessment of the baby is done.  At discharge, the parents are provided with information regarding various topics related to the baby's health using a set checklist to ensure all information is given to the parents and their questions addressed. This includes information on: Vaccination Breastfeeding Nappy change Cord care Jaundice  Formula milk, if the mother asks Bathing the baby Temperature control for baby Vision and hearing Cardiac health Circumcision, if applicable Any other questions or concerns that the mother or caregivers may have.  A follow-up visit with the paediatrician is scheduled at the time of discharge. I have an egg-allergy, would the flu-vaccine be safe?Although some forms of flu vaccines contain trace amounts of egg, the amount is not sufficient enough to trigger an allergic reaction in those who have egg-allergy. The flu vaccine should still be used. Are any additional measures required of me? Frequent hand-washes using soap and water. Alcohol-based hand rubs can be used, alternatively. Avoidance of close contact with those who get sick. How can I play my part in reducing the spread?If you get infected, it is imperative to adopt appropriate measures to prevent the spread of the disease in the community.  To prevent the spread via respiratory secretions/droplets, you must place the inside of your elbow over your mouth and nose when coughing or sneezing. You may have to skip work or school. This is done till you’re off a fever for at least 24 hours, without having to use fever-reducing-medications. Those working in healthcare settings may need extra time off if they’re persistently symptomatic. There are available antiviral medicines that may help protect you from the flu, however, their use is not always appropriate for everyone. Additionally, they are effective only when taken early in the course of the infection. Timely vaccination helps prevent infection, and subsequently serious flu-related complications! How to Use Inhalers

Inhalers are of two types.    MDIs (Metered Dose Inhalers) and  DPIs (Dry Powder Inhalers).  It is utmost important to use these correctly. If you are prescribed MDIs, then DO NOT use these directly into your mouth. Instead, ALWAYS use these with SPACERS. Place the inhaler into the spacer. Put the mouthpiece of the spacer in your mouth. Press the inhaler ONCE to spray the medicine into the spacer. Take FIVE slow smooth deep breaths in and out of the spacer.  Once done, remove the spacer from your mouth and continue normal breathing.  After two minutes, repeat the same steps. It is always advisable to use BEFORE meals. If you are using DPIs the technique is different.  Place one capsule of medicine into the appropriate device. Puncture the capsules in the device. Exhale deeply. Place the mouthpiece into your mouth. Inhale as quickly and forcefully as possible to pull all powder medicines deep into your lungs. The technique of inhaling medicines from a disc-type inhaler is the same as DPIs, a deep forceful quick inhalation.  Remember to use MDIs with slow deep breaths and DPIs with quick forceful inhalation. Your obstetric team shall discuss with you the possible options for delivery of the baby. Vaginal birth is considered a safer option because it incurs fewer risks to the mother and the body recovers quickly. Although some mothers find the idea of vaginal birth after the demise of the baby as distressing, it is the best option if safely possible.  During your stay at the delivery suite, your husband/relative can stay and support you while our experienced midwife will also be there to help you. Finding out about the demise of an unborn baby is always devastating news and experience. Your obstetric team shall discuss with you the possible options for delivery of the baby. Vaginal birth is considered a safer option because it incurs fewer risks to the mother and the body recovers quickly. Although some mothers find the idea of vaginal birth after the demise of the baby as distressing, it is the best option if safely possible.  During your stay at the delivery suite, your husband/relative can stay and support you while our experienced midwife will also be there to help you. Possible pain relief options that we offer. We endeavor to ensure good pain relief for you during these testing times. You can try relaxing techniques like deep breathing exercises, massage, listening to music, or Quranic verses; however, we understand that relaxing at this time is easier said than done. Based upon your general health and medical history we can advise you to give strong painkillers (opioids) or epidural. Your wishes and latest practice guidelines shall be considered before finalizing a plan for you. Strong opioids: This group of medications has a strong pain relief effect. However, they can at times make you feel dizzy and nauseated. We might also have to keep observation on your blood oxygen levels as a precaution. Some patients might feel itching as a side effect. Epidural: An epidural is the most effective methodology for pain relief. A consultant from our team of anesthetists shall use a needle to put in the epidural space in your back. The needle shall be promptly removed while a thin little pipe shall remain in place and deliver the medication during labour. The tube inserted in your back will be connected to the epidural pump for delivery of the medication. We use a combination of local anesthetics and opioids to numb the pain fibers carrying sensation from your belly and birth canal to the brain. Your safety is of utmost importance and hence the anesthesia team will take into consideration your medical history and current medications. An important and recent test needed, before we give an epidural is a complete blood picture profile. We need to see platelet count which has to be more than 80 to administer the epidural analgesia. Once the plan is finalized it takes almost 15 mins to place the epidural and it takes another 20 minutes to be fully effective. As with all interventions in medicine, there are some risks involved. Your blood pressure can come down so we will have to give you some fluids intravenously and frequent monitoring of your blood pressure. About 1 in 100 ladies may develop a headache after epidural placement that is self-limiting but sometimes may require simple treatment measures. Your legs might feel heavy during the infusion. The movement will be slightly difficult owing to the attachment with the infusion pump. A urinary catheter may need to be put in whilst you have the epidural infusion.
